Volunteer Coaches. Existing PVSchools employees need to fill out our volunteer application to ensure
compliance and proper record keeping.

All district level volunteer/chaperone applicants need to fill out a Criminal Affidavit and be fingerprinted.
The Criminal Affidavit can be provided and notarized (at no cost) at the district office. Fingerprinting may
be accomplished by one of two methods:
PVUSD Human Resources: (15002 N. 32nd Street Phoenix, AZ 85032)
- Fingerprint fee is covered by PVSchools
- Must come to the district office to register online with DPS and schedule an appointment for off-site fingerprinting. This process takes approximately 30 minutes.
- Fingerprint results typically are obtained in 2-3 weeks.
- Walkins available from 9am-3:30pm (Monday - Friday) - (Wait times may be longer during lunch hours)
- All applicants and sites will receive an email notice upon approval.
Arizona Department of Public Safety (https://www.azdps.gov/services/public/fingerprint)
- Volunteer/Chaperone applicant is responsible for fingerprinting fee (Approx $80)
- Must obtain an IVP card (a Level One card is not sufficient for our approval process)
- Once the card is obtained, provide a copy to HR so your application can be processed.
- Your IVP card will be valid for 6 years.
